Pelvic floor consists of muscles, fascia and ligaments that form a platform supporting pelvic organs - bladder, uterus and intestines. Pelvic floor prevents these organs from falling down through the lower bony pelvic outlet. Pelvic floor muscles help to prevent urinary and faecal leakage caused by the increase in abdominal pressure during coughing, sneezing, laughing, lifting or vomiting. Pelvic floor muscles also play an important role in sexual activities and can heighten sexual awareness and sensitivity. During childbirth, pelvic floor relaxes and stretches to enable the baby to be born. Pelvic floor muscles are
endurance muscles that play important role during normal daily activities,
such as walking, running, bending or lifting, thus enabling women to lead normal
healthy lives and participate in different activities. It is therefore very important that women maintain strong pelvic
floor muscles by doing regular workouts known as pelvic floor exercises or Kegel
exercises. You can learn more about strengthening of your pelvic floor muscles
on Educational resources, books and guides on Female Continence. The power of posture in pelvic floor health and continence. Kegel Exercisers for strong pelvic floor. Neuromuscular Stimulators for pelvic floor recovery and strength. When pelvic floor muscles weakness develops, pelvic floor begins to drop, bringing down with it one or more pelvic organs.
Genital Prolapse Genital prolapse commonly affects older post-menopausal women, but it also occurs in women of childbearing age. Genital prolapse is a general term that encompasses few different conditions:
A prolapsed uterus occurs, when the uterus drops from its normal position. The cervix may also descent into the vaginal opening. Approximately 35% of women suffer with prolapsed uterus. There are many reasons for uterine prolapse. These include childbirth, long term constipation and obesity, uterine fibroids and the reduction in estrogen levels during menopause. Woman suffering with prolapsed
uterus may feel pelvic heaviness, bulging or fullness and a back pain. They can
also experience vaginal and/or anal pain, incontinence or difficulties in
urinating, bladder infections and excessive vaginal discharge.
2. Cystocele - dropped bladder Cystocele occurs when the bladder bulges down into the space towards the vagina. Bladder prolapse makes implying of the bladder and controlling urinary leakage more difficult and less efficient. 2a. Cystourethrocele - prolapsed bladder with prolapsed urethra Bladder prolapse is often accompanied by prolapsed urethra, when the urethra becomes loose and gapes open. Loose urethra is most often associated with declining levels of estrogens during and after menopause.
Rectocele occurs when the rectal walls become weak and unsupported and bulge into the vagina. Rectocele may be felt as fullness in the perineum or a large bulge inside the vagina, when the rectum is full of stool. A great deal of discomfort is often felt. 4. Enterocele - dropped small intestine Sometimes the small bowel may detach from its supports and fall through the vaginal vault, blocking off the lower rectum. Obstruction of rectal content, feeling of a 'lump' coming down into the vagina, pressure and discomfort are the most common symptoms of this type of prolapse. Please note that the pelvic floor and prolapse illustrations come from the book by Dr Robyn Nagel and Mrs Shirley Owen, entitled The Good Bowel Habit - Pelvic Floor Function and the Bowel - Managing Constipation and Incontinence.
Common symptoms of Genital Prolapse Symptoms of genital prolapse can be similar, but more severe than those experienced with earlier stages of pelvic floor weakness. Symptoms of prolapse include a feeling of bulging, pelvic heaviness or fullness, pelvic pain or low back pain. The symptoms normally worsen with activity and get better with bed rest. While standing, women with a prolapse may feel like something is falling out. Other symptoms can include heaviness or pain in the vagina, urinary incontinence or difficulty urinating, frequent bladder infections, frequent urinary track infections, difficulties with bowel motion, inability to control wind, faecal urgency or frequency and faecal incontinence.
If you are experiencing symptoms for the first time, you should talk to your General Practitioner (family doctor) or a practice nurse. He or she may perform a pelvic examination to assess your condition and decide on the best course of action. You may be referred further to see a specialist, such as gynaecologist, urogynaecologist, urologist or a physiotherapist. A number of different options and solutions can be implemented, depending upon the severity of the prolapse, your age and/or your desire to have children. The prolapse treatments include: - Pelvic floor exercises / Kegel exercises to strengthen pelvic floor muscles. Pelvic floor exercises help to reduce urinary incontinence, improve sexual function and relieve pelvic discomfort.
Please note that you should not stop your pelvic floor muscles strengthening and toning routine (Kegel exercises) just because you wear the V-Brace by Fembrace! For many women deemed not suitable for pessary or surgery, the V-Brace by Fembrace offers a long term solution and an effective non invasive daily relief from the severe problems caused by genital prolapse. - Pessary to alleviate more severe symptoms of the prolapse. A pessary is a special ring in different shapes and sizes that is placed into the vagina to support the prolapse and relieve the symptoms of urinary incontinence. - Surgery is used to repair and reconstruct the weakened pelvic floor and restore its normal function. Surgery is normally indicated with the prolapse is causing significant symptoms and when conservative non-surgical treatment measures have failed.
